The State Health Plan dental and health fee schedules are available to download in Excel file formats. Log into My Insurance Manager^SM  to download the fee schedules. Once you have logged in, the fee schedule links are located under the Resource tab. Follow the links. You will first see the physician network agreement information, then the terms of use. Once you've accepted the terms, you can download the files to your computer.
