The Medical Forms Resource Center (MFRC) gives you convenient electronic option for submitting prior authorization/precertification requests. You can use it for select service types.

The tool gives you all the forms you need to submit complete requests. So, it helps minimize the need for follow-up calls. You can use it to request prior authorization/precertification for services in the following categories:

• Chemotherapy
• Durable Medical Equipment
• Home Health/Hospice
• Hospital Admissions/Inpatient
• Long-Term Acute Care (LTAC)/Skilled Nursing Facility (SNF)/Rehabilitation 
• Maternity
• Medications
• Office
• Outpatient
• Student Health Notification

Even better, when you use MFRC, your request gets priority processing.
 
You can check the status of your requests through My Insurance Manager. To check the status, log into My Insurance Manager. Then, go to the “Patient Care” tab. Under “Health,” select “Authorization Status.”
